  Application for Commercial Replication

HOW TO APPLY FOR A COMMERCIAL REPLICATION PERMIT
  • Secure OMB Application Form for Replication Permit and list of requirements from the frontline receiving/assessing area.
  • Submit the duly filled-up and notarized application forms with complete requirements to the Registry Receiving/Assessing Officer for checking and assessing of corresponding registry fees.
  • Pay the required fees by presenting the Assessment Slip and cash and/or check payment to the Cashier
  • Present the Official Receipt and return the Assessment Slip to the Registry Receiving/Assessing Officer for the issuance of duplicate as Applicant’s copy
  • Claim processed permits from the Registry Releasing Officer after ten to fifteen (10-15) working days or shorter provided that the requirements are complied with. Present authorization letter with photocopy of valid ID if the person claiming is not the applicant.

REQUIREMENTS FOR REPLICATION PERMIT
  • License as Local Licensee.
  • Duly-filled up application form in duplicate copies per title (notarized).
  • Notarized Affidavit of Undertaking.
  • Certified copy of Contract and/or Certificate from the source of rights (notarized).
  • Certified copy of MTRCB Classification Certificate. (for titles with video content).
  • Payment of the following Fees:
    • Replication Fee (per title) – P200.00
    • Recordation Fee (per title) - P100.00
    • Verification Fee (per contract) - P500.00

  • Submission of copies of the approved format of finished product (after replication).

ADDITIONAL LICENSING REQUIREMENTS FOR REPLICATOR (Licensing Unit)
  • Certified List of Machine with serial no. brand name, type of machine, no. of units
  • Photo of Interior of Reproduction site showing the machine/s set-up
  • Flow of Program or Production for VCD/DVD Replication/ Mastering Process (from Raw Materials to Finished product)
  • Certificate of SID Code from Philips International; or
  • Notice of Allocation of SID Code from Optical Media Board
